Helpful questions to ask your insurance provider:

• Does my plan include outpatient mental health or psychiatry benefits?

• Do I have a deductible, and has it been met?

• What is my copay or coinsurance for psychiatry visits?

• Are there limits on the number of covered visits per year?

• Is a referral or prior authorization required?

Rates and Self-pay Options

• Initial Psychiatric Evaluation: $295

• Follow-Up Medication Management: $150

Limited reduced self-pay rates may be available based on individual circumstances. Eligibility is assessed on a case-by-case basis.

Clients who are not using health insurance to pay for care will receive a Good Faith Estimate at least 3 days before your scheduled appointment or upon request.

Payment

All major credit cards are accepted.

Payment is due at the time of service unless otherwise arranged.

Cancellation Policy

If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice. Appointments canceled with less than 24 hours’ notice may be subject to a $50 late cancellation fee.
